Enclosed is your current provider certificate; please note that the last four digits of your approval number have changed to reflect the expiration month and the expiration year of your provider status, and the exact dates of approval are listed below in the “approval period” section on the certificate. All of our providers expire on the 30th day of the month except for February, in which a provider’s expiration date will fall on the 28th. Please be sure to use this current number for all publications and certificates for this event, as using an expired provider number many delay the certification renewal process for the counselors using your services.
